Community Health Workers (CHWs) are the lifeblood of Zufall Health’s public-facing efforts, serving as trusted advocates who help connect individuals and families to the care and resources they need to live fulfilling lives. By identifying and engaging underserved populations, CHWs build meaningful relationships that support our community members. While improving access to primary care remains their primary focus, a culturally responsive approach addresses social determinants of health, such as food assistance, housing resources, transportation services, insurance navigation, and language support.
Through their direct engagement with patients, CHWs play a critical role in improving health outcomes and advancing health equity. They coordinate preventive screenings and vaccinations, support individuals managing chronic conditions such as diabetes and asthma, and help patients stay on track with medications and treatment plans. Our Community Health Workers also provide essential support during transitions of care, helping patients recover safely after hospital discharge and reducing barriers that can lead to poorer health outcomes.
The impact of our CHWs extends beyond individual patients to the healthcare system as a whole. By helping people access the right care at the right time, CHWs reduce unnecessary emergency department visits and lower avoidable hospital readmissions.
As trusted members of the communities we serve, our Community Health Workers are a proven resource in creating healthier populations, reducing costs, and building a more equitable healthcare system.
